Subject: PHARMACY OPERATING PERMITS
A. POLICY
The Hospital Pharmacy as an agent of the UAMS Medical Center shall maintain current permits as required to purchase, store and dispense drugs.
B. PERMITS MAINTAINED BY THE PHARMACY
1. Hospital Pharmaceutical Services Permit
The hospital pharmacy and the OPC pharmacy, UAMS Medical Center, must annually renew the Hospital Pharmaceutical Services Permit issued by the Arkansas State Board of Pharmacy. Standards for qualification are contained in the Arkansas State Board of Pharmacy, Laws and Regulation, and in the Arkansas Department of Health, Rules and Regulations for Hospitals and Related Institutions in Arkansas.
2. Drug Enforcement Agency Permit
The hospital pharmacy shall maintain a current DEA Permit for the UAMS Medical Center. The permit shall authorize the requisitioning, storage, distribution and dispensing of Schedule II, III, IV and V Controlled Substances. In addition the DEA permit number shall be used by medical interns and residents when they prescribe Scheduled drugs in the process of their training programs at the UAMS Medical Center.
The OPC Pharmacy shall maintain a separate DEA permit for use in requisitioning, storage, distribution and dispensing of Schedule II, III, IV and V Controlled Substances.
3. Alcohol Permit
The hospital pharmacy shall maintain a current alcohol permit for the purchase of tax free alcohol. The permit is issued by the Bureau of Tobacco, Alcohol and Firearms.
