Subject: STANDARD DRUG ADMINISTRATION SCHEDULE
A. Policy
The Standard Drug Administration Schedule is established as a guide for uniform dosing of inpatients. This schedule will be used unless the prescriber has stated in writing a specific dosing schedule or if nursing professional judgement indicates a change from these time for a specific patient. Where deemed necessary, first doses may be given as "now" and following doses adjusted to the standard schedule over a period of three doses.
B. Standard Drug Administration Schedule
Daily 0900
B.I.D. 0900 & 2100
T.I.D. 0900, 1600, & 2100
Q.I.D. 0900, 1300, 1700, & 2100
Q 12 Hrs 1000 & 2200
Q 8 Hrs 0000 , 0800, & 1600
Q 6 Hrs. 0000, 0600, 1200, 1800

C. Adjusting Patient Doses to the Schedule
1. All large volume I.V. doses will be hung within an hour of being ordered unless a specific hang time is written and will follow the schedule initiated by the rate of the I.V. and the time the fluid is hung.
2. I.V. antibiotics will be hung within an hour of being ordered unless a hang time is specified. The subsequent doses will be adjusted to the standard drug administration schedule over a period of three doses as demonstrated on the medication administration scheduling guide windows sheet. A copy of this sheet is attached to this policy.3. All oral medication will be scheduled for the next regularly scheduled administration time after the order is written unless an order is specified for "now". If the drug is given off-schedule, then the Medication Administration Scheduling Guide Windows will be used to convert to the standard drug administration schedule.
