Subject: FLOORSTOCK
A. POLICY
A small number of drugs and pharmaceutical supplies are stocked on the nursing units and in the clinics for patient use as needed. These items are limited in number and scope. Lists of the floorstock for each nursing unit will be maintained by pharmacy and approved by the Pharmacy and Therapeutics committee.
B. PROCEDURE
1. Floorstock will be kept to a minimum in variety and quantity of each item.
2. Only items that do not lend themselves to individual patient charging will be considered for floorstock. If it becomes feasible to place an item on a patient charge basis, the item is removed from floorstock.
3. Quantities of each item are adjusted periodically to reflect usage trends. This is done in the two following ways.
a. The nursing staff of the unit or clinic can request increased stock based upon the current usage rate. The Assistant Director for Medication Safety pharmacist or designee will check recent usage and determine whether the request is justified. If so, the standard on the floorstock list for the unit is adjusted.
b. Periodically, the records of filled floorstock are reviewed for usage. Standards are adjusted depending upon the usage.
C. AUTOMATIC REPLENISHMENT OF FLOOR STOCK IN THE HOSPITAL
1. Floorstock is handled by an automatic replacement system.
2. On Tuesday and Friday, the designated pharmacy assistant in the stockroom inventories the floorstock on each unit. A replacement order is generated by subtracting the inventory from the standard.
3. The floorstock order is pulled for each unit.
4. A pharmacist checks the floorstock orders before they are delivered.
5. A pharmacy assistant delivers the order and places the items on the shelves in the nursing unit. Old stock is pulled forward and new or longer dated items are placed behind.
6. A separate list of items in gallons is made every Tuesday. These items are delivered at a later time on Tuesday afternoon or Wednesday morning by pharmacy and/or central sterile supply personnel.
D. EMERGENCY REPLENISHMENT OF FLOOR STOCK
1. Orders of small amounts of floorstock can be ordered by the unit at any time if the usage of an item suddenly increases and the supply on the unit is depleted.
2. Orders are placed on a charge ticket for record purposes.
a. Identify the nursing unit
b. Name of item
c. Quantity of item
d. Mark "floorstock" across the ticket.
3. If the item requested is on the unit floorstock list the order may be filled and sent to the unit without charge.
4. The filled order sheet is sent to the floorstock area so that the quantities can be added to the monthly order sheet for that unit.E. REPLENISHMENT OF FLOOR STOCK IN CLINIC
1. Floorstock is replaced in clinics by the outpatient pharmacy technicians.
2. Floorstock requisition forms are sent from clinic to pharmacy.
3. Charged floorstock (used entirely for a specific patient) is replaced using Clinic Encounter Forms or a prescription.
