Subject: BLOOD DERIVATIVES
POLICY
Pharmacy will stock commercially available blood derivatives as deemed appropriate by the P&T committee. These products will be handled in such a manner to facilitate identification of patients receiving products which may be recalled.
I. The lot numbers and expiration dates of blood derivative products will be recorded on a log upon receipt.
II. All blood derivatives will be stored by lot number in bulk stores.
III. As stock is released to pharmacy distribution, the date of release is recorded on the log.
IV. Blood product recalls will be handled in accordance with policy 9:09 Drug Recalls.
V. When blood derivative products are in short supply, alternative vendors will be contacted to obtain any available allotment.
VI. Appropriate medical staff will be alerted to potential shortages of the various products so that alternative therapy can be used.
VII. Where demand for a blood derivative in short supply is heavy, guidelines for handling the shortage will be instituted by the P&T committee and the Director of the Medical Staff.
