Subject:   REPACKAGING

POLICY

All drugs repackaged in the pharmacy will be handled in a standardized manner.  Individuals packaging the drugs will be familiar with all procedures used in the repackaging process and will be responsible for adhering to them.

B. PROCEDURES

    1. Pull the prepackaging record card and record the product data. If a card does not exist, it must be made before the product can be packaged.
    2. Assign a new expiration date in accordance with policies.
    3. Assign a UAMS lot number to the product.
    4. Record data and the assigned lot number on the prepackaging log.
    5. Select the container or packaging material needed as indicated on the prepackaging card.
    6. Prepare the label in the computer in accordance with the label format guidelines. A sample of the original label is to be attached to the back of the prepackaging card.
    7. Package the drug.
    8. a. Always wash hands before packaging any drug.

      b. Wear gloves when handling oral solids with the strip packager

    9. Check the packages for proper seal. If the container is not self-sealing (ie.. a bottle) apply a "Celon" band over the cap as a tamper proof indicator.
    10. Label all units or containers.
    11. Destroy all extra labels. If the strip packager is used, "Cancel" the label.
    12. Place the completed packages in the prepackaging quarantine area together with the prepackaging card and the empty manufacturer’s containers. A pharmacist must check the packaging operation for drug identity, lot number, strength, expiration dating, quantity packaged and the yield.
    13. Clean all equipment used for packaging the product.
    14. After the product passes the pharmacist’s check, it may be placed in the regular storage area for the product.
