The "GUIDELINES FOR USE" have been developed through a collaborative effort of members of the medical and surgical specialties and the pharmacy department. Each set of "GUIDELINES FOR USE" has been subjected to a final review and approval process by the Pharmacy Committee before publication.
The purpose of the "GUIDELINES FOR USE" is to establish a standard for drug monitoring (DUR, DUE, etc). Drug monitoring is a requirement of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
Subsequent use of the "GUIDELINES FOR USE" during the monitoring process are further subjected to peer evaluation of drug therapies and the specific circumstances and patient needs. Drug monitoring results are used for informational and training purposes. The identity of the provider of care and the patient are held in strict confidence.
