Fosphenytoin Injection

Fosphenytoin Injection

Guidelines for Use

Note: Patients requiring maintenance doses of parenteral phenytoin who have adequate IV access are not candidates for fosphenytoin therapy

It is considered appropriate to use fosphenytoin injection when any of the follow conditions are met:

1. Patients who present to the Emergency Room in status epilepticus or who develop status epilepticus while an inpatient, and other patients requiring rapid loading of high dose phenytoin for therapy.

2. Patients without I.V. access who require parenteral phenytoin therapy (e.g. NPO order, unconscious and unable to take oral meds, active seizures, etc.)

3. Patients intolerant of standard IV phenytoin infusions

* Dosage expressed in phenytoin equivalents (1.5 mg fosphenytoin = 1 mg phenytoin sodium equivalent)

‡ IM or IV fosphenytoin to be used only as outlined above, otherwise, standard IV phenytoin will be used

Monitor:

1. Presence/absence of seizure activity

2. Serum phenytoin concentration

3. Toxicity

a. Transient pruritus (infusion related)

b. Hypotension (infusion related)

Outcome:

Clinical control of seizure activity
