Indications for Use:

1. Infection with culture-proved organism sensitive to drug.

2. Empirical use in presumptive infection commonly caused by gram- positive cocci other than enterococci. (Lab studies, X-Ray, etc. to reinforce decision).

3. Prophylaxis for surgery — duration not longer than 12 hours before or 24 hours after surgery.

Dose:

1 gram every 8 hours normally with maximum of 8 grams daily. Conform to MIC for organism cultured.
