CEFTRIAXONE

Guidelines for Use

Ceftriaxone is considered appropriate to use in the following conditions

  1. Documented culture and sensitivity of non-CNS gram-negative infections susceptible to ceftriaxone and not susceptible to any other antibiotic appropriate for infection site.

  2. Nonpseudomonas gram-negative meningitis in adults

  3. Empiric use in meningitis prior to culture results.

  4. Empiric or documented uncomplicated gonorrhea caused by N. gonorrheae.

  5. Empiric use in suspected serious gram-negative infections unless pseudomonas expected.

  6. Accepted medical practice - stage 2 or 3 - Lyme disease

Dose:

Adults: 1 gm daily (1 gm q 24 hr.) with 1 gram every 12 hours for CNS infections.

Gonorrhea: (uncomplicated): 250mg IM (once) (with oral tetracycline in heterosexuals)

Neonates: 50-75mg/kg/day in equally divided doses every 12 hours- neonate meningitis - 100mg/kg/day divided.

Monitor:

Adverse effects — diarrhea, (including colitis), rash, superinfection with Ps. aeruginosa, Serratia, Candida, B. fragilis, or enterococci

Outcome:

Clinical improvement or eradication of organism noted in patient chart.

Defervescence within 72 hours.
